Examples of PII: name, Social Security number, address, email and date of birth.

Collection, Use and Sharing

We collect certain PII about you, but only when that information is provided by you or is obtained by your authorization. Examples of sources from which we collect PII includes:

* interviews and phone calls
*letters, facsimiles, and email
*website request forms, census forms
*applications for enrollment and changes

We are the sole owners of the information collected. We only have access to /collect information that you voluntarily given us via email, regular mail, fax, or other direct contact with you. We will not sell or rent this information to anyone.

We use this information to determine eligibility for Qualified Health Plans(QHP), affordability programs and other licensed insurance products. We will share this information with appropriate insurance companies, The Health Insurance Marketplace and any other necessary organizations to fulfill your request.

Access and correcting data

Each individual whose PII is collected and stored has the right to access that information in a simple and timely manner. You will be provided with means to correct any inaccurate information or dispute any inaccurate information. By means of submitting a written request, your information will be reviewed and corrected where appropriate. Legal documents such as driver's license, marriage certificate, or passport may be needed to provide as proof for the correct data.

As per PPACA law, the information must be stored for the greater of 10 years or as long as you remain a client of The Benefits Design Group, LLC. Any data that is deemed inaccurate will be properly destroyed.

Security

We take precautions to protect your information. When we collect and store your information it is protected both online and offline. All information is locked up and restricted to appropriate personnel or encrypted. When needed the transmission of information is transmitted in a secure manner. All electronic devices are password protected and/or stored in a secure environment.

All staff and Business Associates are trained in the Privacy Policy procedures. An annual risk assessment is conducted to identify any issues regarding our Privacy Policy and Procedures.

Should a breach occur, whether suspected or confirmed, you will be notified. All appropriate parties including but not limited to The Center Medicare and Medicaid Services (CMS) will be notified. If you feel a breach has occurred, please notify us immediately at (804) 428-0000.
